Registration Package 2010/2011 Season

Hello Hockey Families,
Please review all of the information printed on your child/children's registration and correct any errors you may find.

This year with the computerized system in place to release players, a paper form is no longer required. Please be aware that player movement between lateral or same size centers is controlled by computer and needs approval from our zone manager of operations, zone director, releasing and receiving hockey associations. If you know your family resides in a questionable area of Vulcan's zone, this needs to be indentified at the outset.

Your child has been placed at their age-appropriate level for registration. If you feel that this not what level your child should be playing at, please include a letter stating where they should be placed and the reasons why the VMHA should consider movement. Please note that an overage approval must go through local, zone, and Hockey Alberta before it can be complete and can take some time to put in place.

Registration deadline is AUGUST !%th 2011, for all players registering in teh divisions of Pee-wee, Bantam and Midget. For players registering in Atom, Novice, Pre-Novice and Timbits, registration deadline is SEPTEMBER 15 2011. Registrations received after that date will be subject to board approval and a $50.00 administration fee.

Fee Schedule
Tim Bits (ages 4-6)-----------------$150.00
Pre- Novice (ages 5-6)-------------$250.00
Novice (ages 7-8)-------------------$350.00
Atom (ages 9-10)--------------------$400.00
Pee-Wee (ages11-12)-------------$450.00
Bantam (ages 13-14)--------------$500.00
Midget (ages 15-17)----------------$550.00


Payment
Payment can be made by personal cheque, cash, or through Kidsport (where applicable).
Cheques made payable to Vulcan and District Minor Hockey.
There are 2 payment options: payment in full
                                           post- dated cheques- minimum 50%-dated September 15 2011
                                                                        - remaining 50%-dated October 31, 2011

NSF Fees
A $25.00 NSF fee will be charged on all returned cheques.

Refund and Cancellation Policy
Refunds will be based on the date of written notification to the VMH treasurer. Monies are held back to cover administration costs and applicable fees from Hockey Alberta including insurance. Remaining fees are prorated to cover the cost of ice time and referees. The refund rates are as follows:
Prior to November 1st-------80% of annual fees less $20.00
Prior to December 1st-------60% of annual fees less $20.00
Prior to January 1st----------40% of annual fees less $20.00
Prior to February 21st-------20% of annual fees less $20.00

Birth Certificates
If your child is new to hockey or moved into our association from another association, a photocopy of his/her birth certificate is required. The registrar must provide this information with the player's registration to Hockey Alberta.

Parent Declaration Form
If your family has moved into our community or your child has played for another association, a Parent Declaration form must be completed and enclosed with the registration. This form is available from the registrar or can be downloaded off the Hockey Alberta website at www.hockey-alberta.com

Coaching Applications

Referee Clinic

Code of Conduct Agreements
Please make sure the agreement in your registration packages is signed by both parent and player. If you have a younger child, please read over it with your child and have them sign as best they can. Failure to acknowledge code of conduct agreements will result in registration not being processed. Please remember Vulcan arena is a zero tolerance arena.

Freedoms of Information Policy- please make sure this waiver is signed.

Vulcan Minor Hokey Association Handbook and/or Association By-Laws
A copy of the association handbook and association bylaws are available to all registered members of the VMHA. Log on to our website at www.vulcanminorhockey.com to view.

Hockey Conditioning Camp
Will be held in September, dates to be announced.

Outfitting Your Player
Equipment will include:
-skates
-shin guards
-protective guards
-hockey socks
-hockey pants
-chest protector/shoulder pads
-elbow pads
-hockey gloves
-neck guard
-helmet with face mask
-stick
-mouth guard- MANDATORY- as per VMHA policy
Goalie equipment will be provided by VMHA for Timbits to Pee-Wee level. ALL EQUIPMENT MUST BE CSA APPROVED.
